Japan Air Lines Flight 123 (1985): The Deadliest Single-Aircraft Disaster in History
On the evening of August 12, 1985, Japan Air Lines Flight 123, a Boeing 747SR-46, departed Tokyo's Haneda Airport on a routine domestic flight to Osaka. On board were 524 people—509 passengers and 15 crew members. What began as an ordinary one-hour journey soon became the deadliest accident involving a single aircraft in aviation history.
The Incident
Just 12 minutes after takeoff, while climbing to cruising altitude, passengers heard a deafening explosion. The aircraft suddenly suffered explosive decompression after the rear pressure bulkhead failed.
The blast ripped away a large section of the aircraft's vertical stabilizer (tail fin) and severed all four hydraulic systems. Hydraulic power is essential for controlling the aircraft's elevators, rudder, ailerons, and spoilers. With every hydraulic line destroyed, the pilots completely lost normal control of the Boeing 747.

Captain Masami Takahama, First Officer Yutaka Sasaki, and Flight Engineer Hiroshi Fukuda fought heroically to save the aircraft.
Using only engine power, they attempted to steer the crippled Boeing 747. The aircraft repeatedly climbed and descended in huge oscillations while banking uncontrollably across central Japan.
For 32 terrifying minutes, the crew battled an aircraft that was almost impossible to control. Despite their extraordinary efforts, the damage was simply too severe.
The Crash
At approximately 6:56 PM, Flight 123 crashed into the mountains near Mount Takamagahara in Gunma Prefecture, Japan.

The Cause
Investigators discovered that the disaster was caused by an improper repair performed after a tail-strike incident seven years earlier.
During the repair, the aircraft's rear pressure bulkhead had been incorrectly repaired with an insufficient number of rivets. Over thousands of pressurization cycles, fatigue cracks slowly developed until the bulkhead finally ruptured.
The explosion destroyed the tail section and all hydraulic systems, leaving the pilots with virtually no means of controlling the aircraft.
Legacy
The tragedy transformed aviation maintenance standards around the world. Airlines introduced stricter inspection procedures, improved repair documentation, and enhanced structural fatigue monitoring to prevent similar failures.
